Your First Dental Visit
When you first arrive, you’ll be asked to fill out any new patient forms if you have not done so already. After waiting for a few minutes in our front office, you’ll be called to the operatory to have a full series of X-rays completed and an exam performed by Dr. Rouhani himself. If you need treatment, we’ll explain why and the process in more detail, then connect you with our treatment counselor to discuss your insurance benefits.

New patient forms give our dental office a chance to learn more about your medical and dental history prior to your treatment. The more we know, the better we can personalize your treatments and cater to your specific needs. If you’d like to save time during the check-in process, please download our forms online so you can print and fill them out before your appointment.

Dental insurance is designed to make paying for your dental treatments easier and more affordable, whether it’s routine checkups and cleanings or dental crowns to restore damaged teeth. We gladly accept all PPO plans and are more than happy to help you understand your coverage in better detail.
